Ishmail was arrested in Pennsylvania amidst a mental health crisis, and the mental health specialist at the county jail said he should be sent to the hospital for psychiatric care. Later, a doctor cleared him to return to jail, and within hours, was subject to police violence that led to loss in consciousness and troubled breathing. He was rushed to the hospital, and died 5 hours later, with a brain injury due to lack of oxygen.
